Description
The School Based Behavioral Health Liaison (SBBHL) program provides Masters level mental health clinicians to assist schools in implementing universal, trauma informed practices that prevent and/or mitigate the effects of trauma for students with a Social Emotional Disturbance (SED).

The SBBHL program provides support and consultation to teachers and school personnel in implementing schoolwide trauma-informed practices. It also offers training and education to school personnel in behavioral health topics and trauma-informed approaches.

Description
This program provides high quality intensive in-home services that directly improve the health and well-being of victims of crime with priority given to victims of child abuse, domestic violence, sexual assault and services for underserved victims. We intend to respond to the emotional and physical needs of crime victims, assist to stabilize their lives after victimization, assist victims in understanding and participating in the criminal justice system and provide victims of crime with a measure of safety and security.

Participation is free and voluntary to families. A wraparound team provides support and teaches families to advocate for themselves and utilize both formal and informal supports in the community.

Description
The primary purpose is to successfully transition identified individuals to community based housing by providing short-term intensive support services; our partnering Community Mental Health Centers will develop, implement, and monitor this Initiative's programming. In direct partnership with the state owned regional mental health institutes, the local teams will ensure individual, family, and housing provider support while connecting and coordinating with natural and formal supports within the individual's home community.

Essential Functions of Tennessee Move Initiative Local Teams:

Work closely with state hospital staff to engage long-term individuals ready for discharge * Work in close collaboration with treatment provider to coordinate care * Assist in securing benefits and resources
Work in collaboration with state hospitals in securing housing if needed * Connect to community resources (Peer centers, Psychosocial, Supported Employment, etc.) * Coordinate care with all providers

Description
The Tennessee Healthy Transitions Initiative is a partnership of state and local community agencies and youth led organizations working together to assist Tennessee youth and young adults ages 16-25 who have or at risk of developing a serious mental health condition or co-occurring disorder in improving their health and wellness, leading self-directed lives, and reaching their full potential. By increasing public awareness and outreach efforts and providing effective and engaging services and supports, the Initiative aims to keep youth and young adults from "falling through the cracks."

This statewide initiative provides mental health and other support services (including supported employment and education and peer support) and creates opportunities for informal peer support. Informal peer support is offered in youth and young adult councils, which are comprised of youth and young adults ages 16-25 to years old. Description
The Intensive Long-Term Support program provides intensive long-term, wrap-around support services that are community-based and long-term recovery oriented. Onsite services include psychiatric, nursing, case management and treatment services, as well as living skill development and community activity participation.

These facilities are designed to allow earlier discharge of service recipients from Middle Tennessee Mental Health Institute (MTMHI) and Western Mental Health Institute (WMHI). This program maintains the service recipients in the community in supportive living facilities and provide services which supplement existing services funded by various departments of the state (case management, outpatient psychiatric services, mobile crisis services, peer support centers, etc.) that are not sufficient to meet the specialized needs of this population.

Housing support, on-site psychiatric services, case management, day program and other needed services are provided.

Description
The Family Connection program provides families the tools they need to maintain children and youth with complex needs at home, in school, and in the community. The intensive in-home program is family-driven, providing assistance in navigating the child-serving systems, advocacy, support, and therapeutic skill building to prevent placement outside the home to a higher level of care. Program staff ensures that caregivers are an integral part of the intervention at all stages.

TVC is currently an active partner in federally funded system of care site in the System of Care Across Tennessee project. Our system of care projects may differ in the age of populations they serve but all operate with the same core values:The system of care should be family-driven and youth guided,with the needs of the child and family dictating the types and mix of services provided.The system of care should be community-based, with the focus of services as well as management and decision-making responsibility resting at the community level.The system of care should be culturally competent, with agencies, programs, and services that are responsive to the cultural, racial, and ethnic differences of the populations they serve.

Description
Core to TVC's mission, the SFSN provides valuable support, information and training to parents and caregivers across the state, empowering them to successfully navigate the complex child-serving systems to obtain the services necessary for their children and youth with emotional and behavioral disorders. SFSN staff provides direct assistance, support groups, information and skill-based trainings, family representation on over 150 councils and coalitions, Youth in Action Council facilitation, and outreach to schools, mental health providers, and policy-makers in TN.

Description
The Early Childhood programs promote the positive social and emotional development of young children. It succeeds in its efforts through a cross agency collaborative professional development system, including community based training, continuing teacher education, and higher education. This collaboration fosters and sustains the state-wide high-fidelity Pyramid Model integrated with other relevant Tennessee efforts. The Pyramid Model, as developed by the Center for the Social and Emotional Foundations of Early Learning (CSEFEL), encourages universal supports to all children to promote social and emotional wellness, targeted services to children who need more support, and intensive services to children who need them.
