Description
Abe's Garden Assisted Living is designed for older adults who want to continue an active, independent lifestyle but need a little extra support each day. This program offers a robust social calendar that is built around residents' interests, individualized care, a close-knit community, convenience services, beneficial amenities, and assistance with things like medication management and hygiene.
Program Successes
Abe's Garden Assisted Living maintains a very high level of occupancy. Residents report high levels of satisfaction and are given frequent opportunities to voice concerns to management. "Before Mom moved to Abe's Garden Assisted Living, she went to the Emergency Department three or four times a year. Now, the nurses on staff can evaluate questions right away. She has only been to the Emergency Department once in the four years she's been at Abe's Garden Community. The biggest change in my life is that I know Mom is surrounded by people who care about her and are available 24 hours a day. She has a button on her wrist that she can push at any time, and someone will be right there."

Description
In November 2017, Abe's Garden Community Group (AGCG) launched Middle Tennessee's first ongoing early-stage dementia engagement and support program to address the unmet needs of individuals at the start of their dementia journey. With two groups meeting twice weekly, AGCG is a small group program providing socialization, engagement activities, and support for those in the earliest stages of dementia. AGCG provides participants with a supportive, engaging, small group setting with their peers. Together, group members experience invaluable moments while sharing stories from their past and present; navigating the emotions, questions and challenges that come with their diagnosis; learning new things; enjoying body and brain health-focused activities; bonding; and having fun! Discussions and activities are guided by the members' life stories, capabilities and interests and may include music therapy, tai chi, songwriting, outings, and more.
Program Successes
Group members and family members report high levels of satisfaction with the program. Group members have formed a strong social connection and have voluntarily discussed their diagnoses without prompting. Family members provide feedback, often sharing success stories and gratitude for the respite time it provides for themselves. A participant's daughter says, "My mother feels that she belongs in AGCG. It's a place that makes sense to her and where she feels comfortable and valued." She adds, "I've been grateful that she has had a place to go with others who are sharing similar experiences. It has kept her mind engaged and she enjoys the people and the activities. I know that she is a valued member of the group and it is a relief to see her in a place where people are happy to see her and she feels important."

Long-term Success
The long-term goal of AGCG is that individuals who are in the early stages of Alzheimer's and other dementias will feel hopeful about their future, continue to enjoy hobbies and activities, and maintain physical, emotional and cognitive abilities longer than otherwise. The group will reduce the stigma surrounding dementia, and allow individuals with a diagnosis to be open about their disease and continue with their lives without embarrassment over their condition.
Short-term Success
The program members enjoy social connections and an opportunity to speak freely about their diagnosis and struggles. They enjoy a range of activities including learning new skills. Tailored exercise classes and cognitive games may sustain, or even improve their physical and cognitive abilities, while their emotional well being may improve due to being part of a close supportive social network.
Program Success Monitored By
Residents are asked for feedback regularly, and their preferences inform programming. Family member feedback and the transition of members to other Abe's Garden Community programs when appropriate indicates success. The program is overseen by a dedicated Program Manager and a Licensed Social Worker. Enrollment and retention rates also act as a measure of success.

Description
Abe's Garden at Home has a customized approach to in-home care and engagement for seniors who need a little support to remain independent at home. Our care partners receive special training that helps them uncover each person's unique needs, abilities and interests. They then create personalized opportunities designed to enhance the quality of life through support with self-care tasks and mind and body wellness activities, all while providing respite for family caregivers. With a goal of helping families remain in place, Abe's Garden at Home assists older adults with engagement and with the activities of daily living. Our at-home services are often an alternative to an assisted living community or skilled nursing facility. Our services include assistance with:

Shopping, medical appointments and errands

Program Successes
We often hear caregiver express concerns that their loved one just sits and watches TV and that it's difficult to engage them day in and day out. They often assume their loved one would not be interested in participating in any introduced activities, but quickly witness the Home Engagement Coordinator's ability to find the right activity to engage the client.
"I was worried how Jim would react to having a young woman taking care of him every Friday afternoon- I need not have been concerned at all. Catie is always cheerful, full of projects and ideas to keep Jim thinking and reacting. I was free for four hours with not one concern."

Description
The purpose-built courtyard and surrounding households at Abe's Garden Memory Support feature evidence-based design and house 42 individuals. Daily life unfolds according to individual preferences, not predetermined schedules or care partner convenience, and residents have opportunities to participate in purposeful engagement programming based in the I'm Still Here(R) approach to support self-expression and interactions with others. Nationally recognized aging design professionals and lighting industry researchers contributed to our architecture, landscape and interior design. Created to encourage independence without creating stress or intentional distraction, features include three households, each featuring a distinct theme: Music & Movement, Connection to Nature, and Arts & Lifelong Learning. The kitchens and dining areas are the hub of each household, so residents can see, smell and participate in meal preparation.
Program Successes
The program maintains a wait list that regularly exceeds the number of residential places available. Families report high levels of satisfaction with the care their loved ones are receiving. Industry professionals are routinely impressed when visiting campus to witness the program. Resident are largely happy and satisfied, with low occurrence of reactive behaviors commonly observed in individuals with dementia. Quotes: "I will never find the words adequately express my gratitude to you and all the staff at Abe's Garden for how you cared and loved my mother. You enabled her to spend her last year of life with happiness, dignity and love." "My mom sat in her house for three years before coming to Abe's Garden under what we thought was our great care. Long-term Success
The long term goal of the Abe's Garden Memory Support is to be a model for high quality memory care, resulting in an elevated standard of residential memory care across the globe. Individuals with dementia will not struggle to find high quality care in their local area, resulting in an improved quality of life for them and their family caregivers who are often burdened with trying to cope at home, or by guilt due to placing their loved one in an inappropriate setting. The stigma surrounding Alzheimer's and dementia will be reduced, and those diagnosed with these disorders will continue to live with dignity, purpose and enjoyment.
Short-term Success
Abe's Garden residents will be happy, safe and cared for, and their families will be secure in the knowledge that they have done the right thing by placing their loved one in memory care. Areas of care that require improvement will be quickly identified, and will show measurable improvement over the course of targeted training.
Program Success Monitored By
Our Quality Improvement team assesses the quality of all aspects of care given. When areas in need of attention are identified, then they become the focus of routine training, and subsequent observations ensure the maintenance of high standards. Family feedback and demand for residential places also provide information regarding the success of the program.

Description
Abe's Garden Club is an on-site day program for individuals living at home in the early to mid stages of dementia who may benefit from the exceptional care and top-notch engagement activities offered to residents. On any given day, Club members might enjoy creating art inspired by other cultures, singing to reminiscent songs, exercising on a recumbent elliptical machine, making cookies, harvesting tomatoes, arranging flowers, tapping along to the drum circle, writing greeting cards, or playing with the resident cat. Abe's Garden Club offers flexibility for our participants through a variety of monthly membership plans designed to meet families' needs, including three or five days of programming each week.
Program Successes
Families of Club members report high levels of satisfaction, and Club members appear to transition more easily to residential care than those who have not previously been members of The Club, "I am so pleased that she has some structure to her week and is being engaged in activities with other people. It's difficult to question her about her experiences due to her memory issues, so I was thrilled when she brought up Abe's Garden Club and said she likes it!"

Long-term Success
Club members will experience a more fulfilling life by having the opportunity to fill their days with social and engaging activities rather than being at home every day with their caregiver. This also provides family caregivers with time to rest or accomplish non-caregiving tasks while the Club member is at Abe's Garden Community. Club members will more easily transition to residential care if and when it's needed, as they are already comfortable in the Abe's Garden Memory Support environment and have existing relationships with staff and residents.
Short-term Success
Success is seen by engaging Club members with purposeful activities to improve their quality of life. They are surrounded by their peers in a social, home-like environment.
Program Success Monitored By
The number of individuals who eventually transition from the Club to residential care is an indication of success, as is family and Club member feedback. As with residential care, the Quality Improvement team observes the quality of care being provided and implements training and observations focused on areas requiring improvement.
