Description
Faith Family Medical Center staff provide medical care for working, uninsured and underinsured individuals.
Program Successes
One of our recent success stories was featured in our social medial. One of our patients, Debbie M. had been diagnosed with Diabetes. Debbie utilized all of Faith Family's resources to educate and empower herself about her chronic disease. She is up to date on her annual requirements and has been able to manage her glucose levels. She is now hopeful about her future.

Description
Through the Journey to Wellness Program, Faith Family Medical Center targets the prevention and management of health risk factors. By offering free individual consultations, and group wellness classes on nutrition, fitness, diabetes education, smoking cessation, and stress management, Faith Family provides patients with the tools to improve their health and decrease their risk factors for diseases and conditions such as diabetes, heart disease, cancer, high blood pressure, high cholesterol, and obesity.
Program Successes
Sonya came to Journey to Wellness (J2W) for the first time after her medical provider suggested she meet with our Wellness Director, Beth Allen, for some advice on a healthy diet and exercise plan. At that time her cholesterol was elevated and Sonya really wanted to try to make some big changes before going on medication. She met with Beth and took all the information to heart. In five months, she has lost 39 pounds and feels much better. She is drinking lots of water and incorporating fiber-rich, fresh veggies into her diet. Sonya is walking every day and has an active job cleaning offices, which helps her reach her daily step goal.

Description
The women of Tennessee are consistently ranked among the unhealthiest women in the country. One of the most effective ways to change this sobering statistic is by providing women access to primary healthcare. Through Faith Family's Women's Health Initiative, we seek to provide medical screenings to women who do not have health insurance. By offering such services as PAPs, blood pressure, cholesterol, mammogram referrals, on-site mammograms through Saint Thomas Health's mobile mammogram bus, and other necessary tests, low-income women will have the benefit of prevention and early detection of any medical issues so that they may receive the information and treatment they need to enjoy optimal health.
Program Successes
Maria S., a local customer service representative, came to FFMC for recurring breast pain. She was uninsured and could not afford a regularly priced mammogram. She met with her FFMC provider who did a clinic breast exam and referred Maria for a diagnostic mammogram. The funding we receive for the Women's Health Initiative enabled Maria to received the mammogram at a price she could afford. The mammogram was normal and the patient will continue with annual screenings.

The programs at Faith Family meet our mission of serving the working uninsured and other underinsured by treating them physically, emotionally and spiritually. Our providers are fortunate to be able to spend time with each patient so that they can perform a comprehensive assessment of the individual. This may include all aspects of the patient's life: their family and support systems, their spiritual life, their fitness level, and their health literacy. Faith Family is blessed to have a robust pharmacy program (through Lipscomb University) that helps patients get the medication they need. The Journey to Wellness program is led by a registered dietitian who can meet with patients one-on-one or in a group setting. She provides education regarding their nutrition and physical fitness.
