Description
The SEED Program is a nation-wide program developed by people with disabilities for people with disabilities, keeping with the STRIVE4You peer to peer approach. This program is anchored by experienced leaders with over 30 years of martial arts and conflict resolution training, as well as 12 years' experience teaching people with disabilities self-defense. This skilled based program takes a holistic methodology to exploring preventative safety, consent & boundaries, threat analysis, appropriate threat response, and the emotional effects of a traumatic situation, while at the same time promoting exercise & fitness. Allowing qualified students to become the teacher allows the SEED program to expand into communities around the country maximizing the overall impact!

Description
The ARISE program from STRIVE4You was developed to provide fun, innovative, and informative opportunities to educate our community. These opportunities provide information on how people with disabilities can participate in recreational activities, competitive sports, and physical education classes alongside their peers. STRIVE4You believes that bodies in motion stay in motion, allowing people to develop healthier and more productive lives. Too often people with disabilities are excluded from participation in their favorite activities. We want to change this by providing the knowledge to create a more inclusive, level playing field for everyone. STRIVE4You provides local adaptive sports & recreational events, hands-on adapted sports workshops, professional development opportunities, an annual adaptive sports conference, and one-on-one consulting. Not only do we serve people with disabilities, but those within the community that provide services to this population.

Long-term Success
Goal 1: STRIVE4You will introduce people with disabilities to adaptive sports and recreational activities, increasing their overall activity level and sense of confidence and empowering them to live more productive, healthier lives.
Goal 2: STRIVE4You will increase people with disabilities' knowledge and methods for advocating their athletic needs to others.
Goal 3: STRIVE4you will provide educational opportunities for physical education instructors, coaches, community sports and recreation professionals, and others to ensure inclusive communities where people with disabilities have opportunities, facilities, and local resources to live healthier lifestyles.
Goal 4: We will offer one-on-one consulting to professionals working with people with a disability to provide action plans for effectively including a person with a disability in mainstream activities such as physical education classes, after-school athletic activities, or community-based wellness programs.

Short-term Success
Goal 1, Objective 1: We will increase the overall activity level of 25% of participants by 25% in the first year.
Objective 2: 40% of individuals in the first year will experience a 40% increase in their feeling of confidence due to their participation in adaptive sports and recreation.
Output 1: Six hands-on workshops will be provided over a one-year period for seventy-five people with disabilities and fifty professionals.
Output 2: Four Adaptive Sports Days will be provided over a one-year period for fifty individuals.
Output 3: In 2020, we will host an annual adaptive sports conference with multiple adaptive sports during National Disability Employment Awareness Month in October for fifty people with disabilities and twenty-five professionals.

Goal 2, Objective 1: 50% of attendees will increase their knowledge of adapted sports by 50% in the first year.
Output 1: Four webinars will be provided in the first year for twenty participants each.
Program Success Monitored By
To measure the knowledge participants, gain at the workshops, we will use a pretest and posttest designed survey.To measure the objectives from Goal 1, surveys will be filled out by participants three months after the workshop or Adaptive Sports Day to evaluate how their activity and confidence levels have changed since attending their respective events.

Each child that participates in PE with a Twist will be provided a letter describing the activities they participated in to discuss with their parents, along with a questionnaire that will evaluate their understanding of people with disabilities.

All data collected will be stored in a database and will be reported in our annual report.

Description
A disability should never define who you are, nor should it dictate how people approach and interact with you. However, each and every day, hundreds of thousands of people feel they are treated differently, and they feel isolated and frustrated because of the lack of equality and inclusion throughout society. Negative stereotypes, unequal opportunities, and a simple lack of disability awareness in our communities are major barriers for those of us who strive for acceptance and equal opportunities for all. our BE REAL program provides education, resources, and support to contribute to the solution. First, we help children with disabilities learn essential life skills such as self-advocacy, communication, and Job readiness to prepare them for a productive life. Secondly, we provide peer-to-peer support to adults that were born with a disability or developed one later in life. As they navigate challenges, we are here to make sure they know about all of the wonderful resources available.

Long-term Success
It is our goal to offer peer-to-peer group discussions, online courses, workshops, and interactive conferences. Online courses include for youth a Personal Finance Management course, and an online course geared to businesses that implore or have customers with disabilities to improve disability awareness. Likewise workshops for these businesses and for people with disabilities will be held to cover similar topics. Support groups as well as one on one, peer to peer, consulting will be provided to help gain knowledge of resources and develop personal plans of development.
Short-term Success
During 2020 STRIVE4You will provide Zoom based webinars and support group meetings. We will also offer Professional Development workshops teaching disability awareness with companies throughout our communities. Once fully developed we will launch online courses in Personal Finance Management, Communication & Advocacy, and Job Readiness. In 2021 we will offer our first BE REAL Conference, an interactive three day retreat for transition age children wherethey will participate in hands on life simulation exercises to build Extended Core Curriculum (ECC) skills, while giving their parents an opportunity to network and learn resources available to their families.
Program Success Monitored By
To measure the knowledge participants, gain during events/courses, we will use a pretest and post-test survey. Each event/course will have a registration application that must be completed prior to participation. This will help with collecting participation data. Additionally, online course participants will be required to complete assignments and test to evaluate knowledge gained from the course, with a 85% or better overall grade needed to successfully complete the training and receive a certification of completion. Peer to Peer and support group participants will also complete a customer satisfaction survey. Individual progress will be documented through case reports.
