Description
When someone is faced with a terminal diagnosis, the last thing they need to be worried about is where they are going to spend their final weeks and days, or who will be caring for them.
Often times, the burden of care has no where to fall. We have found that we can do a lot with very little. We are fortunate to be able to supplement our care with trained and willing volunteers.
Providing the necessary care to our residents costs $430 per day, which includes 24/7 caregivers, nourishment, laundry services including clothing needs and linens being provided, and the organizational costs to run the home. The average length of stay for our residents is 8 weeks.
A sponsorship of $25,000 would provide 8 weeks of dignified end-of-life care to persons who would otherwise be left in the hospital or be discharged to an unsafe environment without proper care.
There are an average of 7 (8 week block) opportunities to be a sponsor, each block costs $25,000.
Program Successes
Death is not the ultimate tragedy of life. At the end of the 8 week block there would be an 80% occupancy rate in the home. That would mean a minimum of 3 residents were served at a length of stay of at least 15 days during each 8-week block. Remaining consistently fully staffed and having the ability to stretch our staff into other areas, will allow us to achieve this goal and also increase the number of residents we serve in a year's time. With this success of the program it would also mean a larger outreach. Administrators would then able to go out into the community to educate physicians, clinicians, case managers, etc... on how to access care at the house.

Long-term Success
Long term success of the Sponsor a Resident Program will mean that we are serving as many people in our community as possible when they are faced with a terminal diagnosis and in need of housing and dignified care. We also achieve success when we have relieved the suffering of our residents​, lifted a caregiver burden​, improved caregiver well-being​, and exceeded care expectations.
74%​ of Americans who are caregivers are aged 75+ and they are also caring for someone who is aged 75+. (AARP and National Alliance for Caregiving) Long term success of this program will result in a better quality of life with access to equality in healthcare with a hospice​ team. With services provided regardless of the ability to pay​, success also means that we have pulled in a strong sense of community involvement​ through the generosity of donors and volunteers. Serving the dying poor in our community is a necessary function of a flourishing society.

Short-term Success
We have the capability of serving 3 residents at once. Most of our occupancy has been with 1 resident at a time due to being understaffed. When the house was full with 3 residents, the home was filled with so much joy, but it was extremely challenging to meet the needs of our residents, especially when our intention is to give them the very best that we can. When we are fully staffed and present to all of our residents, we see their emotional needs decrease, their pain lessons, they are not as anxious, and they sleep and eat better. Short term success is measured by the attainment of the following goals, which are also recommendations of the ​Institute of Medicine's report Dying in America​; encourages comprehensive medical and social services that are​ patient centered​, family oriented​, accessible around-the-clock​, consistent with individuals' goals, values and informed preferences​ and collaborative across organizations. Program Success Monitored By
Success of the Program will be measured by the occupancy rate of our beds in 8-week intervals. The first measure would be filling our beds at least 80% of the time. Not only would we want to fill the beds, but we would also want to provide adequate one to one care, which would leads to our second measure; assuring that staffing goals are met. Lastly, this would be measured by our waitinglist. Although we want to serve every referral we receive, that is not possible when beds are full and occupancy rate is high. However, the data of a growing need would speak volumes when assessing for the measured needs of adding a 2nd CCCH. If long term success is achieved, then we also open the door to the possibility of home #2. Overall success is of course measured by community involvement and continued education in end of life care and the growing need of equality in care.

Description
Home Modifications Fund: We currently serve our residents in a 900 sq ft basement. We have two semi-private bedrooms, a half bath, and an open concept living room and kitchen. Our washer and dryer is upstairs. One of the bedrooms does not have a permanent wall separating it from the living room (we have curtains), therefore, in order to maintain a quiet environment, we only use the far side of that room for resident quarters, the side on the curtain end is used as a sitting room. Our kitchen has a small sink, microwave, and refrigerator. We utilize a hot plate and countertop oven to prepare meals. Our goal is to raise $150,000 by 5/31/2024. We plan to have 3 to 4 private bedrooms, a full bathroom with a walk-in shower, a full size washer and dryer downstairs and a kitchen with proper amenities.
Program Successes
An example of program success is attaining our goal, in time as well, and being able to complete the needed modifications in a timely manner that is the least disruptive to the residents. The greatest success would be that we are able to attain lump sums throughout this year, in order to complete the work in phases, as to not disrupt to so much of the day to day flow of the care we provide.

Success of this program would not only enhance the aesthetics, but would also make the function of the house more efficient. The first priority being that our residents would have their own room. More than giving them space, we want to give the resident who may be actively dying the dignity of privacy, as well as to give respect to the resident who may be completely aware of their surroundings and knows that their roommate is passing on from this life. f they have family, this will also provide a safe and comfortable space for the family to come together in private. Storage is pretty much non-existent in the home currently. broadening the storage possibilities would not only serve our residents well, but our staff and volunteers would have better ease of function. Not many of our residents are capable of showering, but for those who are, having the ability to provide a hot shower when needed, would be incredible! A full functioning kitchen and laundry facilities is imperative.
Short-term Success
Short term success will be motivating for the house as a whole. It will show all those involved in the house and in the community that we are all serving our residents well! We can all be assured that the community is accompanying this mission and the intentions of the house are being heard and supported. If we were to meet our goal sooner than May 2024, we could start on the renovations sooner, which would prove a tremendous short term success. The short term program success would strengthen the relationship we have with St. Edward Church, who are supporting us as a charity of their parish. It would allow HOMH to put down roots and begin to incorporate other members of the community, such as contractors, electricians, plumbers, Architects, etc...
Program Success Monitored By
The program Success will be monitored by residents having an even greater presence of peace and tranquility, which in effect result in having fewer symptoms that need to be managed so often. Volunteers have a greater sense of ease when navigating the home and attaining storage solutions for items used. It also opens the space up to more people from our community to visit or volunteer, and most importantly, encourages family and friends of the resident to feel more at home and have more extended space to be together. As funds grow and this program is gaining the funds needed, this will encourage and inspire others in the community of the importance of having a space that is even more welcoming to the dying poor, their family, and the volunteers and caregivers who serve them in the home.
