Mission

The mission of the Primary Care & Hope Clinic is to improve the health and well-being of the communities we serve by providing excellent and compassionate care, accessible to all.

Background

In 1992, a group of concerned citizens conducted a needs assessment of health care in Rutherford County, including access and affordability. The results of the survey showed that while there was an adequate number of quality providers, access to these providers was limited for individuals without health insurance either due to unavailability or lack of affordability.In 2012, the health center achieved Federally Qualified Health Center (FQHC) designation. The health center is located in a 27,000 square foot facility, the Community of Hope, which was completed in September of 2008. The $6.5 million dollar building project was completely funded through donations from the community including 2.4 acres of land from the City of Murfreesboro and $3.9 million from the Christy Houston Foundation. In December 2015 we completed a building renovation at our main site to accommodate our growth and continue to increase access to health care services for the people we serve. The project was funded in part by a Christy Houston Foundation award. The project added nine patient rooms, a pharmacy, and additional administrative and support offices. The center operates through support from federal, state, county and local government, churches, private foundations, and individual donors. The Primary Care & Hope Clinic provides a comprehensive program designed specifically to meet the medical needs of the uninsured. These needs encompass not only the physical examination, but provision of lab work, x-ray diagnostics, behavioral health services, medications and social services.

Impact

In the last year, the Primary Care & Hope Clinic served more than 12,000 patients, providing more than 42,000 patient visits. In 2022 we completed the purchase of the building where our Shelbyville clinic and pharmacy are housed, and in 2023 we completed the expansion of services at that site. In June 2023, our new 20,000 square foot campus in Smyrna opened. This project will allow us to expand services to the Northern Rutherford County area, offering primary care, behavioral health care, pharmacy, dietary, and care coordination.

Goals for this year: Enhance the customer experience, increase access to services for the underserved population, enhance patient safety, maintain financial stability and continue to seek grant funding and increase fundraising efforts to support the mission of the Primary Care & Hope Clinic.

Needs

The top 5 needs for the Primary Care & Hope Clinic are: Funding for the increasing number of uninsured individuals requesting healthcare services. Funding for additional mental health services. Funding for additional social services and case management services. Funding for improvements to our Shelbyville campus to expand services to the Bedford County community.

CEO Statement

The Primary Care & Hope Clinic is a non-profit health center providing quality, affordable family health services to uninsured, Medicare and Medicaid families, and the privately insured. It was developed in 1992 by a group of concerned citizens who saw a need to increase and improve access to health care. I joined the health center in 1996 as the CEO and also serve as a Family Nurse Practitioner. Every day I meet people who are trying to provide for their families. Many of these individuals are unable to afford insurance because they have encountered diseases or accidents that have changed the course of their lives. I consider it an honor to have the opportunity to help them make the transition from facing a health care crisis into becoming self-sufficient.

Board Chair Statement

Access to quality, affordable healthcare is something that many of us take for granted. A growing number of our neighbors are facing the reality that they are unable to obtain medical insurance coverage due to unaffordable premiums. Statistics show that uninsured patients fail to obtain preventative health care, as well as primary care for routine illnesses, because they have no consistent point of health care access. Too often, the lack of access causes uninsured patients to develop advanced diseases and seek treatment in the most expensive, accessible environments. The Primary Care & Hope Clinic seeks to strengthen its mission of providing accessible, affordable health care for the uninsured and underserved through the combined efforts of volunteers, businesses, educational, civic, religious, professional and governmental institutions. By involving the entire community, the clinic can operate as a responsible steward of its charitable resources while providing efficient and clinically excellent health care for the uninsured and underserved. It is my privilege to serve as Board Chair and extend the mission of "hope and healing for all".
