Description
TNBC operates several satellite programs in order to reach low-income women in all corners of Nashville. TNBC makes this mobile program available to women through partnerships with other nonprofits, such as St. Luke's Community House, Bethlehem Center, the SE Branch of the Nashville Public Library, Gilda's Club and the McGruder Family Resource Center. These partnerships allow TNBC to reach more women in the Nashville area who may not have the means to travel to TNBC's Craighead Street location. Similar to our signature 12-week program, the programs at these locations include strength and conditioning training, nutritional education, and personal empowerment classes that focus on healthy choices for nutrition, activity and exercise, body image, and self-confidence. Our personal trainers travel to these facilities with all the necessary materials and equipment to ensure that these clients receive the same level of education offered at the main center.

Description
The program provides physical fitness, nutrition education, and self-actualization classes for women, regardless of income, for 1 year. This program allows women to gain the holistic knowledge and skills needed to build and sustain a healthy lifestyle long-term.
In the area of fitness, each client is assessed individually by a Certified Strength and Conditioning Specialist and has fitness and nutrition programs designed to meet their wellness needs. Clients meet in 24 sessions with the coaches. Body weight, BMI, and improvements in health markers are measured and tracked. Self-image, nutrition, and healthy living habits surveys will be taken before and after the 24 sessions to measure progress.
After the initial 24 sessions are complete, clients continue to attend group exercise classes for 9 months and are encouraged to support each other in complying with the foundational wellness principles of the program. Coaches continue to monitor clients and follow up outcomes are measured.

Our records indicate that since 2013, 75 percent of the women who graduate from each year-long program have reduced their BMI by 2 points in one year. This indicates a total reduction of incremental healthcare costs of $133,775 per year return on investment for each of New Beginnings' programs in one year. Another way to describe this significant reduction in health costs is to consider that graduates of the TNBC program have reduced publicly funded medical costs by $1,023,975 for the 550+ women served this past year.
Results include weight reduction of 7%, BMI reduction of 2 points, over 40% of women stopped taking prescription medication for diabetes and hypertension or reduced their dosage, 92% tested improved self-concept and confidence, 93% increased physical activity in their daily lives, 90% experienced a decrease in pain and physical limitations, 100% improved their strength and mobility. These results indicate significantly lower risks of chronic diseases.
Short-term Success
Short-term outcomes are measured in person on a daily and weekly basis during the 12-week portion of the program. Coaches measure clients' weight, BMI, and body composition. In addition, they administer the World Health Organization's WHO 5 Well-Being Index, which measures quality of life for 5 items on a 6-point Likert scale. A score of 0 represents the worst possible quality of life, and a score of 100 represents the best possible quality of life. An increase in score of just 10 points is considered a significant improvement in quality of life by the WHO. Our clients score on average 14 points higher upon completion of our 12-week program. Coaches also administer a nutrition and healthy habit survey pre and post the 12-week portion of the program to measure growth in nutritional knowledge.
Program Success Monitored By
In October 2019, we were given funds to purchase a research-grade body composition analysis machine. It allows us to go beyond body weight as a measurable outcome and accurately measure changes in clients' body fat mass, visceral fat mass, skeletal muscle mass, intra and extra-cellular water, and basal metabolic rate.
Additionally, to better demonstrate the holistic impact TNBC has on clients, we are implementing two new measurement tools. The first tool measures clients' sense of loneliness. Loneliness has been shown to be a greater factor in health than smoking, high blood pressure, and obesity. We have implemented the UCLA 3-Point Loneliness Scale to track changes in clients' sense of social connection and support. Stress is another major contributor to chronic disease. Teaching techniques and systems to manage stress has been a part of our curriculum since we started and the Perceived Stress Scale-4 (PSS-4) allows us to show the impact we are having in this area.

The biggest challenge facing this organization is the perception that all a client has to do to improve their health is to eat less and move more. So this should be easy. The fact is that without the knowledge of and coaching necessary to change unhealthy behaviors, good intentions often fail. People hurt themselves by working out without the knowledge of the proper form and understanding of their current mobility issues. People diet without changing underlying habits, which is why diets fail 97% of the time. Lasting change can only happen when thought patterns change and poor lifestyle habits are replaced with healthy habits. Tennessee currently spends billions of dollars to fight diseases related to obesity, but little to prevent and stop obesity. The New Beginnings Center is offering a new way to prevent and stop behaviors that lead to obesity and the related diseases. Tennessee ranks 39th (close to the bottom) in wellness and it costs our state $2.81billion dollars per year in obesity health related expenditures, an 80% increase since 2008. There is a indirect correlation between income and poor health. The New Beginnings Center currently needs the funding to meet our initial budget goals so that we can begin to serve the hundreds of low and fixed income clients in need of affordable wellness programs that will deliver results. |
