Background
Connectus Health (formerly known as University Community Health Services, Inc.), currently operates two clinics in Davidson County. Both clinics offer primary care, pediatrics, women's health (including prenatal care and attending deliveries at Saint Thomas Midtown),transgender health, and behavioral health. Connectus serves all patients regardless of their ability to pay or insurance status. Vine Hill Community Clinic, the first location established by Connectus Health, is the largest site and has served the community for nearly 30 years.
Impact
Accomplishments received recognition from the Health Resources and Services Administration (HRSA) and insurance carriers as a high quality health center. Increased behavioral health, women's health services, and social services to all patients. Recognized as an Advocacy Center of Excellence by the National Association of Health Centers. Successfully hosted first annual Connectus Back to School Bash providing school supplies and support services for over 350 Davidson County families. Hosted multiple community conversations to identify unmet community needs and support opportunities. Goals: Fully implement Medication Assistant Treatment (MAT) services for substance use disorders at both clinic locations. Increase access to preventative women's health procedures such as colposcopies and LEEPs. Increase access to care for pediatric patients and families through the addition of mobile services.
Needs
As Connectus continues to grow and expand, the need for additional provider and clinical support talent continues to increase. With the success of the first annual Connectus Back to School Bash, Connectus needs additional community partners and donors to help increase the reach of the event and the number of families served. Community partners and donors to support social services provided to patients and families at Connectus such as diapers, food pantry items, and transportation vouchers.With insurance enrollment assistance services no longer receiving federal funding support, Connectus needs donor support for insurance enrollment and education services. Donor support of free medications available to Connectus patients and families.
CEO Statement
Connectus is a non-profit community healthcare organization founded on the values of compassion, wellness, community and culturally sensitive care. Since inception, Connectus has been proud to serve some of the most underserved and highest need populations. In 2018, the organization served over 9,000 patients with 98% of those served at or below 200% of poverty.
Board Chair Statement
Connectus Health (formerly University Community Health Services) is a mission driven organization. Since its inception, Connectus has faithfully served those in need of comprehensive primary health care. Connectus is nationally recognized for providing high quality care to people who face significant financial, language and cultural barriers when accessing care. Many of those we serve struggle to obtain the care they need for themselves and their children. Often our patients have difficulty affording their medications. We are proud of our long and successful history of helping people obtain the care and medications they need. With experienced leadership and board oversight, Connectus continues to grow and expand services to meet the needs of low income uninsured families in our community.
