Description
Launched in 2005 as Bridges to Care Plus, Project Access Nashville Specialty Care provides a coordinated system of care to low-income, uninsured residents of Davidson County, which links patients to a network of community-based medical clinics, volunteer physician specialists, collaborating hospitals, and ancillary partners. Qualified patients are assigned a primary medical home at one of the primary care clinics in the Project Access/Safety Net Consortium of Middle Tennessee network. When a patient requires specialty care, a referral is sent to PANSC. A PANSC care coordinator will verify eligibility, collect medical records, make the initial appointment, and coordinate care with the clinic, patient, and volunteer specialist. PANSC also assists with and tracks any needed follow-up visits, ancillary services (such as laboratory, imaging, and diagnostic testing), and interpreting services. If required, participating hospitals provide services at no charge.

Public health education includes education and outreach to physicians, patients, and the community at large on health topics. The Medical Foundation of Nashville conducts this activity using a variety of methods, such as in-person seminars, flyers, social media, and one-on-one interaction with individuals. Public health education is conducted throughout Middle Tennessee as the foundation provides education to the community on various health topics with the goal of empowering the community to make healthier choices towards increasingly healthy lifestyles.

Description
The Medical Foundation of Nashville coordinates educational opportunities for physicians and physicians-in-training in the form of Continuing Medical Education courses, leadership training, and other seminars supplementing the traditional physician education provided by medical schools on topics such as financial planning, business and medicine, and new laws impacting healthcare. The education is conducted in Middle Tennessee by the Medical Foundation of Nashville in collaboration with other partners with expertise in the field of study.
