Description
Starting in July 2024 CCM will be enhancing services provided to our clients. We have diversified to include wrap-around services. These services are designed to improve the quality of live by providing a variety of soluctions to needs they face. CCM will still provide basic needs for families, children, elderly and such but will be enhancing those offerings allowing for continuing change. We will be partnering with other organizations whose expertise is what we need. The goal is to provide a complete library of solutions for the clients individual needs to promote improved growth for families and improved well being.
Program Successes
Program success will likely be individual since it is individuals that are experiencing difficulties. An example would be if we have a family that is living in their car. The children are not in school, they have no food, they have no understanding of services available and can't read. This is a real issue that is faced. If we can get transitional housing by utilizing our collaboration with transitional housing, many things can happen. First everyone can be fed which will help the children learn when we get them in school or tutored. Second we begin to look for low-cost housing and help them complete the needed forms to apply. Since they will be able to develop personal hygiene practices, we can begin to help with employment options. While they are living at half-way house they are learning social skills, communication skills and learning how to interact with others properly. The conclusion is that all parties have personal growth and well-being and become part pf society.

Long-term Success
The long term success will be determined by monitoring how the lives and well-being of our clients has improved. Our client base lives on the fringes of society, meaning education and skills are limited. They can not understand nor process requirements needed from many social service organizations. In addition their skills for computers etc do not exist so while it may improve the agencies, it leaves many behind. Our goal is to help them navigate the services that are currently available and provide new ones they may need. The ultimate goal is to develop a library of sorts that can be adapted for the individual's needs thus improving their overall physical and mental health. We believe that improved behavior health will be a natural outcome of improved physical and mental health.
Short-term Success
Short term goals are working in collaboration with other agencies that have expertise in particular types of issues. We must listen to our clients to determine what are their root needs. Only then can we begin to form a plan to help. Short term will really be on-going as we develop programs and services for the families, childer and elderly. It is important to note that success will not be overnight but slow and steady progress will be everlasting.
Program Success Monitored By
The executive director and staff will be monitoring the program and success will be provided by feedback from out clients.

CEO/Executive Director/Board Comments
Hunger has no boundaries. Hunger impacts the lives of 1 in 7 Americans. Real people of all walks of life struggle with hunger. It does not matter if you live in a suburban, rural or urban setting - hunger lives in our community. In 1983 16 area churches saw the need to consolidate human service activities for the Madison area. They established Christian Cooperative Ministry, a non-denominational organization to help families in need. For over 35 years CCM has stayed true to the mission and continues today to feed the hungry. Starting in July 2024 we will add wrap-around services for clients that will improve and strengthen their overall growth and well-being. |
