Description
The building blocks to connection and reimagining life outside homelessness starts with relief and community. Within our street outreaches it looks like:
RELIEF - We serve the immediate needs of meals, clothing, and survival supplies for stabilization. We also have robust partnerships that help us provide showers, haircuts, and even pet care.
COMMUNITY - We cultivate trust, dignity, safety, & love through healthy interactions with volunteers and staff.
CONNECTION - Our outreach specialists use this platform of hospitality leading to trust to establish plans and provide support to those seeking permanent housing. We provide direct support, casework, job services, counseling, mental health referrals, medical resource referrals , general crisis navigation, recovery resources etc all leading to dignity through permanent housing.

Our Outreach Specialists serve at all outreaches developing relationships to guide individuals on their journey to Homefullness.
Program Successes
RELIEF:
In 2022, we serves over 50,000 meals & over 30,000 units of clothing.

COMMUNITY:
"I couldn't do it by myself. With your whole group, you pushed me without even knowing it to do the best I could. I owe a lot to Andre. He is the main reason why I sobered up. Seeing the love your team shared every Tuesday night, made me keep pushing." - Gary - Gary is off the streets, he's been sober for over two years, just received visitation rights to see his children, & is in the process of getting a job.

CONNECTION:
In 2022, 38 individuals/ families received housing through the direct support of our team

COMMUNITY:
Through healthy and positive relationships developed at our outreaches, we will continue the work of helping individuals ready to take the next steps needing to get access to the resources that will help them continue on the path to stability.

CONNECTION:
Individuals who choose the Homefullness journey, they will work with one of our Outreach Specialists to assess needs, set goals, and develop a personal plan along with having a community to support them along the way.

Description
Building a Homefullness Journey

Locating The Most Vulnerable: Through our numerous outreaches, we engage & identify the most vulnerable of our city.

Stabilization: Our caseworkers start stabilizing needs for food, clothing, survival supplies, documentation, & process challenges.

Building A Support System: Combining our work with our key partners, we provide a healthy community, physical health support, mental health resources & counseling, education opportunities, accountability, & safety within housing.

Resource Navigation: Documentation, mainstream resources, employment support, financial education, housing navigation, & long-term planning.

Program Successes
We stabilize the immediate needs so we can begin focusing on and supporting each individual who is ready and willing to begin the journey to Homefullness.

With our trained outreach team, in partnership with other local agencies, and our volunteer community, we work to develop a safety net that our clients need in order for them to find stability and sustainable independence in safe and secure housing.

Developing a holistic and individualized plan to walk with someone on their homefulness journey. We will work with each person to create a stabilized living situation in which they can rebuild their lives in a sustained way. We'll help individuals navigate through life's challenges as well as find extended long-team resources to build sustainability, including connecting individuals to key organizations within job services, mental health services, medical resources, crisis navigation, and housing navigation all while promoting healthy community structure.



Short-term Success
Stabilizing the individuals' basic needs - food, clothing, survival supplies; while we begin their process to a long-term solution. Weekly connections with Outreach Specialists creating specific plans for each individual to have the skills and tools needed to live independently in a sustainable way. We'll establish goals with the individual and coordinate with additional organizations to ensure the individual has access to mental health resources, educational opportunities, job placement, medical services, and other needs they may have as we work on their housing navigation and placement.

Description
We aim to meet people in the critical period of first moving into their new home to begin to imagine life after experiencing homelessness.

MIND: Counseling, Goal-Setting, Art, and Equine Therapy.
BODY: Cooking, Cleaning, Organization, Finances, and Physical awareness.
SOUL: Spiritual nourishment opportunities

Until there is a safe space for someone, destructive and unhealthy cycles are hard to break.
We are intentionally providing spaces for the body, mind, & soul to heal. We do this through weekly check-ins involving reflection and goal setting. We are also partnering with counselors & therapists to build therapy & recovery opportunities. We have forged partnerships to provide easy gateways for our neighbors to gain low barrier job training & opportunities including starting our own in house coffee company, Paradeisos Coffee. We find that rebuilding a healthy community is crucial in the long term sustainability.

Currently serving 3 people in our program as of May 2023. Great progress in their journeys of mental health, housing stability, self confidence and vocational development!
