Description
The Food Pharmacy provides fresh produce, shelf stable food and nutritional counseling for chronically ill and oncology patients screened for food insecurity. The Food Outreach Program delivers the same foods to those referred by our partner government and non-profit partners. There are five identified food deserts within Davidson County and Feeding American Identifies 114,000+ residents with food insecurity . These identified patients are given a food prescription based on their personal diagnosis towards low sodium, low fat, low sugar or super foods. While there are plenty of food pantries in our community, the provision is based on whatever is available. These two programs provide only the right foods towards enabling recipients to thrive and sustain wellbeing.

the two programs have provided 415,000+ nutrient dense meals across the community since March 2020.
Short-term Success
The Outreach Program with the Food Pharmacy began after the tornado and COVID-19 struck our community. During 2020, teh Food Pharmacy Program provided more than 200,000 meals across our community.

Description
Low Health Literacy is the degree to which individuals have the capacity to obtain, process, and understand basic health information needed to make appropriate health decisions. 12% of US adults have proficient health literacy. 9 out of 10 adults lack the skills needed to manage their health and prevent disease. the NGHF Health LIteracy efforts are centered around four pillars:
1. Production of general health topics in easy to understand and follow information. English and Spanish and free on website / 19 topics have been completed thus far.
2. Health Literacy Guides have been published for specific disease diagnosis and to community residents. Guides for Oncology in English, Spanish and Arabic and Diabetes in English and Spanish
3. Annual Health Literacy Forum will be offered again in October 2022. Each year the Forum brings national speakers and local health advocates together to address improving health literacy in our community.

Recent publication of Health Literacy Guide for Oncology and Health LIteracy Guide for Diabetes. Both are just released and will be tracked with sets of data on efficacy over the next 12 months.

Description
The Patients Medication Fund represents a bridge fund for patients who are not yet enrolled in existing community medication programs and are unable to afford the cost or the co-pay for necessary medications to manage their well-being.

Description
Following the death of our hospital's Chief of Oncology, Steven Wolff, MD, The Nashville General Hospital Foundation created a special fund in Dr. Wolff's memory--and with his guidance before he died. The Steven Wolff, MD Oncology Patient Fund provides financial resources to oncology patients in need who cannot work or who have lost their jobs due to the rigors of chemotherapy and radiation treatment regimens. The fund has collaborated with the Team Chad foundation to marshal the funds necessary to pay for non-treatment expenses for oncology patients including rent, mortgage payments, grocery bills, transportation costs [from home to treatment and back] as well as nutritional support items. Every patient is quite grateful and is then able to continue with their treatment knowing that they will still have a roof over their heads and food in their refrigerator to feed their families.

Description
African American/Black Men have the highest incidence of prostate cancer in the United and twice the rate of Caucasian Men. While some studies identify genetic variants as a contributing factor, research confirms lack of adequate health insurance, inequitable access to health care, and absence of a primary care provider are barriers to timely prostate screenings and the diagnosis of prostate cancer. As part of this new offering, Nashville General is offering no-cost screening and lab work for uninsured and underinsured men.

The NGH Foundation Board and Staff are proud that our community, donors, volunteers and staff have had the flexibility to pivot during the pandemic to respond to the overwhelming needs of those with food insecurity. More than 200,000 meals have been distributed to those identified by trusted sources. With the impact of the pandemic far from over, we continue to provide the most basic resources - food, transportation, medications to those seeking to manage their health. Our only limitation is funding.
