Description
The Supported Living program provides community based supports to adults with developmentally disabilities. The agency supports 21 individuals to live in their own homes or apartments in the neighborhood of their choice. Typically we provide a live-in Companion to provide whatever supports the individual needs to live successfully and to be fully included in their community.

Description
The employment program helps adults with severe developmental disabilities secure and retain community based employment. Job opportunities are varied and fully integrated. Provided supports are individualized, typically with the provision of a Job Coach to ensure the customer remains successfully employed.

Description
Adults with severe developmental disabilities can receive Community Based Day during those times when they are not gainfully employed. Supports are individualized and fully integrated in the community. To help individuals to be fully contributing, involved members of their local community staff support is provided as needed. People are supported in volunteer opportunities, community leisure activities, retirement activities and more.

Description
Adults with severe developmental disabilities residing with their families can receive Personal Assistance services. The Personal Assistant provides whatever support the family needs so that their loved one with developmental disabilities can continue to live in the family home.

Adults with developmental disabilities and serious health issues receiving Supported Livings Services may also receive Licensed Practical Nursing and Registered Nursing services. Assistance can include the administration of insulin, assistance with tracheotomy needs, etc.

Mid-TN Supported Living, Inc. has a reputation for providing cutting-edge supports and services to adults with Intellectual and Developmental Disabilities. We recognize the importance of building a strong sense of community on behalf of the people we have the opportunity to serve. Many of the adults we support have had limited opportunities to have a meaningful life due to previous institutionalization. We are very excited to report that 32% of the people we support are now homeowners! Over the years, we have placed a growing emphasis on helping people with severe disabilities secure employment in fully integrated settings. Job opportunities and job sites are quite varied. During the last couple of years, the agency has helped three people with severe disabilities to become small business owners! One of the primary components that sets our Day Service apart from many other programs is that supports are exclusively provided in fully-integrated, community based settings. Such a model of support affords persons with severe disabilities the opportunity to enjoy full inclusion and to become valued, contributing members of their communities. Although we embrace the value that all people should have the opportunity to experience the many rewards associated with working, we also recognize the very difficult challenges we face as we continue our efforts to help the most severely disabled to secure and maintain successful employment. Again, we recognize the challenging work that lies ahead and feel it is important to note that we continue our commitment to all persons we support to help them have meaningful lives regardless of their employment status. For many this means employment, for others this may mean volunteer work or other community based supports.
