Description
HOPE's BH program, funded through a HRSA Behavioral Health Integration grant, is a vital source of mental healthcare for patients. Our experienced staff include a Program Coordinator, LCSW, LPC, Psych NP, Community Health Coordinator (CHC) and a dedicated Receptionist/MA. Services include medication management, individual therapy and EMDR. HOPE also offers BH consults, I&R, and limited case management through the support of our CHC, able at a moment's notice to step in with primary care patients at the request of a medical provider. By offering brief assessments and targeted referrals within the primary care setting, we are able to get patients on the path to healing without having to independently request a mental health assessment. In one visit we can identify prospective patients, affirm their need, schedule an appointment, and introduce them to their provider. From July 1, 2017-June 30, 2018, HOPE's BH providers offered care through 1,916 encounters, with remarkable results.

Description
HOPE has clearly shown the depth of its commitment to the Hispanic community of rural Middle TN through our decision to hire a full-time interpreter and Bilingual Patient Navigator. Our neighbors include hundreds of tobacco, farm and construction workers, many of whom move through life without any formal social support or meaningful personal connection to the English-speaking world. This program offers invaluable support to our many Spanish-speaking patients as they seek care at HOPE. Through the entire process - from registration to triage, from labs to treatment plans - our cherished Hispanic patients receive the bicultural and bilingual support of a qualified interpreter. In CY2017, our Bilingual Patient Navigator served 625 Hispanic patients through 1,460 patient encounters! HOPE's Spanish-speaking patients come to us from great distances, as far as Cookeville, Crossville, and even Pulaski. If that isn't a compliment and testimony of gratitude, we don't know what is!

Description
Many residents of rural Macon, Northern Sumner and Trousdale counties have difficulty accessing the traditional health care system. HOPE seeks to meet the needs of this vulnerable population by using a sliding scale fee schedule to increase the affordability of health care for those otherwise unable to obtain needed interventions. Service for these patients is truly the heart of our mission. Our primary care services include comprehensive physicals, lab services, preventive care, well woman visits, sick visits, treatment of acute illness, chronic disease management, basic procedures, on-site mammography (offered in partnership with St. Thomas), referrals for advanced diagnostic testing & specialty care, behavioral health step-ins, I&R, and more. In CY2017, HOPE's medical providers offered 11,320 medical visits! We are proud to serve over 4,000 of our neighbors, and have recently achieved federal designation as a Patient Centered Medical Home, Certified Level 2!

Description
Through a unique partnership with Engle Family Dentistry of Gallatin, Tennessee, and with the support of a HRSA Dental and Oral Health Services grant, HOPE has expanded its scope of services. Dr. Ken Engel offers comprehensive dental services to patients referred internally by HOPE's providers. Services include dental exams, x-rays, cleanings, deep scaling, sealants, extractions, fillings, crowns, and referrals for dentures, root canals and complex oral surgery. We are excited about this new partnership and look forward to serving our community together.

Description
Thanks to special HRSA funding, the Pharmacy at HOPE Family Health opened July 11, 2016. Staff includes a full-time clinical Pharmacist/Director of Pharmacy Services, a part-time Pharmacist, two Pharmacy Techs and a Patient Assistance Program Navigator. In 2017, HOPE's Pharmacy filled 20,283 prescriptions representing a total 'usual and customary price' of over $1,449,000. However, due to our status as a 340B Pharmacy, our ability to file insurance claims for those who have insurance, and our regular discounted prices and special discounts for those with income below 200% of the federal poverty level, our patients paid $228,215.93. The on-site Pharmacy program and the expertise of our Pharmacists provide an invaluable service for those without transportation, with limited income, and/or with complex pharmaceutical regimens. Many patients qualify for free medication through the Manufacturer Prescription Assistance Program. Last year the PAP saved 233 eligible patients over $1.485,000.
